Do you publish straight to live, or as drafts?

Your call, set once. Most clients start with drafts for the first few pieces and switch to direct publishing once they trust the output. Either way the metadata, internal links and schema are already in place.

What if we use a page builder like Elementor or Divi?

We work inside it rather than replacing it. Posts are created in the format your theme already renders — we are not going to rebuild your site as a condition of publishing to it.
